Sebastian’s Support Service Referral Form

This service is for children & young people between the ages of 8 and 25 who are experiencing long term illness or palliative care or are a sibling of someone experiencing long term illness or palliative care and would benefit from some emotional well being support.

Self Referral or Referrals from Parents / Carers:

Once you have completed the referral form below a member of staff will contact you; each young person’s care will be assessed on an individual basis.

Referrals from Professionals:

Please email referrals@youthworksnorthamptonshire.org.uk to arrange a consultation call with a member of our team.

Data protection agreement

In this form we ask for some personal information, including contact details and information relating to personal history etc. in order to fulfil your request for service with us.

Any processing will be performed in line with the Data Protection Act 1998 and the General Data Protection Regulations.
